Qualifications: BSN degree required or equivalent years (usually four) of relevant clinical nursing experience. Current California RN license required and PHN certificate preferred. One year acute care experience and two years experience in a Home Health or Hospice licensed agency and one (1) to two (2) years supervisory or lead experience. Demonstrated knowledge of The Joint Commission, federal, state and local regulatory requirements. Preferred Qualifications: Bilingual (English/Spanish), Hospice/Home care experience, Med-Surg, critical care, pain management and end of life care. Duties: At one site, responsible for first line supervision of hospice and/or home health nurses and aides and other patient care staff., including interviewing, hiring, monitoring assessments and evaluating personnel. Ensures individualized patient care goals are met. Ensures the highest quality of care is provided and in compliance with federal, state and local regulatory requirements and established departmental policies and procedures, quality/cost/clinical outcomes and established performance standards. Supervises day to day operations and clinical practice standards. Develops and implements action plans to improve staff development, quality and delivery of patient care. Acts as a clinical practice expert/resource to staff. Participates in program management, including licensing, budgeting, utilization and quality assessment/improvement systems, processes and methodologies. Ensures patients receive quality care by reviewing documentation, consulting with staff, reviewing care plans, conducting/participating in multidisciplinary team conferences, and reviewing patient satisfaction reports. With appropriate team members or contracted services, coordinates the clinical care of hospice and/or home health patients and families. Reviews all visits made by contract services for adherence to hospice and home health policies, protocols and standards. Investigates and resolves patient/family/member concerns regarding patient care. Consistently supports compliance and the Principles of Responsibility (Kaiser Permanente's Code of Conduct) by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting with ethics and integrity, reporting non compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licenser requirements (if applicable), and Kaiser Permanente's policies and procedures. In addition to defined technical requirements, accountable for consistently demonstrating service behaviors and principles defined by the Kaiser Permanente Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives. Also accountable for consistently demonstrating the knowledge, skills, abilities, and behaviors necessary to provide superior and culturally sensitive service to each other, to our members, and to purchasers, contracted providers and vendors.
