CASE MANAGER - On-Call position

Description

Conducts regular reviews of the patient's clinical course to determine appropriate treatment and level of care, using InterQual criteria.  Collaborates with the physician, staff, patient, and family to assess discharge planning needs, and coordinates post-discharge care and referrals.  Documents assessments, discharge plans, and actions taken.
Qualifications

Minimun of 5 years of acute care RN experience required, plus recent experience with utilization review and discharge planning.  Current CA RN license and BLS.
