Case Manager - Home Health Registered Nurse

Home health agency in Central California is seeking a Case Manager, Home Health Registered Nurses. In this role, the Case Manager has overall responsibility for the successful management of a patients' case throughout the continuum of care as a patient. Successful management will include positively impacting: Clinical outcomes, patient care plan goals, appropriate utilization of the agency's financial and human resources, and patient satisfaction. Case Managers work a minimum of 72 hours per pay period and generally case manage 20-25 patients.

The Case Manager is responsible for playing a role as the giver of, supervisor of, coordinator of and oversight for the patient's care. This is the prominent oversight role and will intersect dynamically with the rest of the organization during any given case.

Monday - Friday Day Shifts -8 Hours

Minimum Requirements:
•Diploma or Associate Degree in Nursing.
•Minimum of 1040 hours continuous and recent experience as a home health nurse
•Current Basic Life Support (BLS) certificate.
•Must be able to travel
•Must provide DMV printout for last three years driving history.
•Ability to manage own time and schedule own tasks.

Preferred Qualifications:
•Certification in area of specialty.
•Bachelor's Degree in Nursing.
•Public Health Nurse Certificate.
